E-Proof Agent Entry
Welcome to E-proof, the insurance agent online entry for contractors' general liability insurance.
The purpose of this service is to allow you to provide evidence of
general liability insurance for your insured contractor directly to the Construction Contractors Board.
Use of this service replaces the need to submit a hard copy of a
Certificate of Insurance by mail or fax to the CCB.
THIS PROGRAM CANNOT BE USED TO SUBMIT INSURANCE CANCELLATIONS
It is critical that you have confirmation of coverage from the
company or broker prior to representing insurance coverage on this site. Misuse of this service or misrepresentation of coverage could be
subject to disciplinary action by the Oregon Insurance Division under ORS 746.075.
To use this service, you will need the license number or
name of the contractor you are insuring. You will also need your assigned User Name and Password.

Workers' compensation insurance coverage

Cancellation of insurance
For cancellations, insurance evidence for a new contractor,
or workers' compensation evidence, a Cancellation Notice or a
Certificate of Insurance must still be mailed to:
